Your First Visit

From the moment you call and talk to our staff, you will probably notice that we are different from most doctor’s offices.  Our goal is to make you feel comfortable and special from the very beginning. 
So relax, and let us take care of you.

When you come to our office for the first time, we have a simple straightforward
new patient forms that will be easy to fill out. (You may have downloaded your
new patient forms from our website or received them in the mail prior to your visit).

Next, your vital signs will be taken and the staff will get a brief history.
Then Dr. Cudnik will do an in-depth consultation to discuss
 your health concerns and answer any questions you may have.  The next step is a physical examination, including range of motion testing, muscle testing, and neurological and orthopedic testing.  The doctor will recommend and take x-rays if he feels they are necessary to accurately diagnose your case.

Treatment and therapy may or may not be recommended on the first visit.  It depends on you, your condition, and the doctor's opinion.  In most cases you will be scheduled to return for report of findings in a recommended action plan for your relief and recovery.  If x-rays or other diagnostic tests were ordered, they will be explained and reviewed at this time.

Remember, our goal during his first visit is to make you feel at home and comfortable.
 And the doctors and staff will explain and walk you through every step along the way.
